Payment Options

At Active Family Chiropractic and Acupuncture, we never want cost to be the reason that a patient doesn’t get the treatment that they need. This is why our office offers a variety of payment options.

Care Credit

One option that many patients choose to use is called Care Credit. This is an interest-free credit card based upon your credit. You may choose between the first six months are interest-free plan or 24-month interest plan. You need two forms of identification to qualify. If you are interested in seeing if you qualify, please contact our office for more information and to start the application process. Those patients who already have Care Credit are free to use it as a form of payment in our office.

In-Office Payment Plans

We also offer in-office payment plans to fit your budget needs. A debit, credit, or checking account on file is required for automatic payments to be processed on your account according to what plan fits your needs best. These payments can be set up weekly, biweekly, or monthly.

Insurance

Our office is in-network with almost every major insurance company. It is our office policy to pay at the time of service. Any copay, deductible or coinsurance due from your insurance policy would be due at the time of service unless payment arrangements are made.

We have a state of the art insurance department that verifies every insurance and we provide you with your estimated cost of care when you first visit our office.

No Insurance

We offer those individuals without health insurance a 20% discount for paying at the time of service. *Cannot be combined with any other special including new patient exam and x-rays. Does not apply to acupuncture services.*

You have the right to receive a “Good Faith Estimate” explaining how much your health care will cost.

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
  • Make sure your health care provider gives you a Good Faith Estimate in writing at least one business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
  • Make sure to save a copy or picture of your Good Faith Estimate.
